Enlisted on 20 June 1915 at Liverpool, NSW.

Address at Enlistment: Henleage, Milton Street, Mackay, QLD.

Embarkation Details: Embarked Overseas on

16 February 1916 with 13th  Battalion,

14th  Reinforcements at Sydney, NSW on

Ship H.M.A.T. A70 "Ballarat".

Cause of Death: Killed in Action on 19 July 1916 while serving with 54th  Battalion at Somme, France at the age of 27 years.

Burial place: No Known grave.

Next of Kin:  Sister, Mrs. A. G. Evans, Milton Street, Mackay, QLD.

Religion: Presbyterian.
