Embarkation Details: Embarked Overseas on

19 September 1916 with 47th  Battalion,

5th  Reinforcements at Brisbane, QLD on Ship H.M.A.T. A49 "Seang Choon".

Cause of Death: Killed in Action on 5 April 1918 while serving with 47th  Battalion at Dernancourt, France at the age of 36 years.

Burial place: No known grave.

Next of Kin:  Brother, Joseph Fitzpatrick, Balough, County Cavan, Ireland.

Religion: Roman Catholic.
